What's in Your Medicine Chest?
We all take a variety of medicines, vitamins, and perhaps even herbal remedies from time to time. Sometimes it can be difficult to remember them all. But certain medicines and vitamins or herbs can be harmful if taken in the wrong combination.
That is why it is important to keep track of what is in your personal “medicine chest.” Make a list of what you take on a regular basis, what it does and who prescribed it for you. Keep a separate list for each family member. Always review your list with your doctor whenever you start or stop taking a medication, and keep it up to date.
Your system for keeping track of your “medicine chest” might look something like the form below. Click on the picture of the form (below) to print as many blank copies as you need. (If you have problems printing, try using the right mouse button on your computer to "open link in new window.")
Healthcare professionals know it is very important to have complete and accurate information about what medications and supplements a patient has taken before giving out any additional drugs or medicines. The information a patient provides can be critical in making sure that only the right dose of the right medication is given. With a home medication form, it may be easier for you to provide complete and accurate information when you need it most.
